Instructions for Individual Plan of Action Year-End Report

Each Extension agent, county director, program assistant/associate and ag technician is expected to complete an individual plan of action year-end report at the end of the respective program year. This report should be completed after all ERS data has been entered but by January 15th. Information included in the report can and should be used with the Extension Performance Appraisal Tool in evaluating individual performance for the year.

To complete the individual plan of action year-end report, one should go to their online individual plan of action and click on the Year-End Report tab. Information pertaining to each section is as follows:

Your Selected Objectives: Lists the objectives you selected to address that were included in your county plan of work. The Definition and My Details links are shown to allow you to see background information pertaining to the objective as well the particular components you selected to address within the objective.

Your Individually-Reported Objectives: Lists the objectives that you have reported on within the year. A Definition link is once again included. Clicking on the ERS Data link will take you to a page that summarizes the outcome, impact, volunteer, and program cost/benefit analysis data that you entered in ERS during the year for the objective. Also included will be any narratives that were entered into ERS for the objective.

Your Team-Reported Objectives: Lists the objectives that have been reported on where you were a member of a team. Clicking on the ERS Data link will take you to a page that summarizes the outcome, impact, volunteer, and program cost/benefit analysis data that has been entered by the team leader.

Educational Methods: Lists the educational methods you planned for the year. For each method that was completed during the year, check the box to the left of the method to indicate it was completed. Some boxes may already be checked if you previously checked the box by a method in the Individual Plan of Action Educational Methods and Timeline section on the One Stop Shop web site.

Reporting: This section allows easy access to all ERS reports completed by you and others. By clicking on the ERS Main Menu link, you will be taken to the ERS state reports page where you can find and print ERS data entered by you during the year.

Learning: This section allows easy access to the Extension Learning Management System whereby you can view and print the training events you participated in during the year. Clicking on My Report will display your LMS information allowing easy access to your report for the year you select. The People Report link can be used by county Extension directors and district Extension directors to view LMS information for staff members within their respective counties and districts.

Diversity Statement: Lists the ideas for reaching new or underserved audiences that you included in your plan of action. In the text box underneath, you should enter what you did to each these audiences.

Marketing Efforts: Lists the ideas you included in your plan of action for marketing your programming efforts and program impacts as well as your plans for marketing the total county Extension program. In the text box underneath, you should enter what you actually did in your marketing efforts.

Administrative Responsibilities: For CEDs only. Shows those items you listed in your plan of action that you would address under each of the CED competencies. In the text box underneath each competency, enter in bulleted format what you actually did to address each competency.

County Extension directors will have access to year-end plan of action reports for each of their staff members. District Extension directors will have access to year-end plan of action reports for each staff member in their district.
